Nursing Care
Partners In Care Home Care provides specially trained, registered nurses for your home care needs, including ostomies, wounds, incontinence, medication management and IV or feeding tube therapies which can all be managed in the comfort of your home. Nurses, under a physician's direction, work closely with you and monitor all your needs to provide a comprehensive and individualized plan of care.

Rehabilitation Services (Physical, Occupational, and Speech Therapies)
Our team of rehab professionals has over 100 years of combined experience. We come to your home to provide physician-ordered services. We work with persons who are "homebound", or who have difficulty getting to an outpatient clinic. We specialize in addressing home safety issues and fall prevention including:
- Safe mobility, including ambulation and transfers
- Safe self-care activities, including bathing, cooking, and dressing
- Bathroom equipment recommendations
- Information on community resources
- Cognitive assessments
- Other recommendations to make the home setting a safer place
Physical Therapy
A Physical Therapist provides treatments to patients who have limited strength or movement as a result of an injury or illness. Treatment may include exercises and instruction on balance, walking, and transfers. Your physical therapist will facilitate safety within the home environment and may introduce the use of a cane, walker, or wheelchair for greater mobility. We also treat musculoskeletal pain issues.
Occupational Therapy
An Occupational Therapist provides treatment to people who have difficulty performing basic activities of daily living due to an injury or illness. Treatment may include dressing, bathing, grooming, eating, or cooking, as well as exercises to strengthen arms, hands, fingers, or to build endurance. Your occupational therapist will make recommendations for safety with in the home environment, which may include adaptive equipment.
 Speech Therapy
A Speech Therapist provides treatment to people having difficulty with speech, language, or swallowing as a result of an injury or illness. Speech treatment may include activities for speech production, auditory and reading comprehension, writing, and memory. Swallowing treatment may include exercises and recommendations for diet modification to improve function and safety.
Social Work
Social Workers provide support in looking at factors which may affect your independence in the home. Social Workers can give you information about help that is available in the community. This information may include different ways to receive help in the home, transportation options and planning for your future needs, such as advanced directive planning.
Home Health Aides
Our Home Health Aides are often our most valued members of the home health care team. Home Health Aides provide personal care, including bathing and dressing, and they assist with therapy programs. Our Home Health Aides undergo background checks and complete ongoing training in order to provide you with the best service possible.
