The Home and Community Based Services Program has been implemented by our Case Management Team for over twenty years. The program helps older adults develop a plan of care that includes the health, social, and personal assistance services they need to live safely and with a sense of well being at home.
Case Management Team
A social worker and a registered nurse work closely with the client, family, physician, attorney, and local home health and community service agencies to coordinate and monitor a range of services to meet an individual's needs.
