Partners In Home Care is Western Montana's only non-profit, nationally accredited home health care agency. Partners In Home Care was co-founded by Community Medical Center and St. Patrick Hospital & Health Sciences Center. We have been bringing health care home to patients and their families for over twenty-five years. Our main office is located in Missoula at 2687 Palmer, Suite B. We also have a branch office serving Lake and Sanders Counties, located in Polson at Grandview Clinic, #6 13th Ave E. 1-888-729-8848
